Get Your Flu Shot

VNSNY is committed to keeping you healthy and safe, especially during flu season and COVID-19. That is why all staff are now required to be vaccinated against the flu — or, if you choose not to get a flu shot, you must sign a declination form.

Here are the flu immunization regulations from the New York State Department of Health (NYSDOH): “During the influenza season, all healthcare and residential facilities and agencies shall ensure that all personnel not vaccinated against influenza for the current influenza season wear a surgical or procedure mask while in areas where patients or residents may be present.”

FOUR EASY WAYS TO GET YOUR FLU SHOT

To make it as easy as possible for you to get your flu shot, here are four FREE ways to get it:  

1) Obtain a Flu Voucher for Walgreens-Duane Reade or CVS

  • For a Walgreens-Duane Reade voucher, click here.   
    • You can print out the form or download it to your phone. (Walgreens-Duane Reade will accept this form from your phone along with showing your Partners in Care ID Card, so you do not need to print it out.) 
    • When at Walgreens-Duane Reade, remember to ask for your flu documentation as proof of your vaccination for your Human Resources record.
    • The voucher will be emailed to the address you indicated 
    • Print the voucher or download it to your phone. (CVS will accept this form from your phone along with showing your Partners in Care ID card, so you do not need to print it out)
    • When at CVS, remember to ask for your flu documentation as proof of your vaccination for your Human Resources record.

2) Go the Partners in Care office located at 245 East 41st Street, First Floor on Monday’s or Wednesday’s between the hours of 8:45 am and 4:00 pm or by clicking here, you can go to one of these VNSNY office locations. You can also read more about getting your flu shot by clicking here.

3) Visit your doctor, health care provider or urgent care center (most insurers will cover flu vaccinations from a care provider at no cost, but you should check with your insurance company to be sure).  When at your doctor or health care provider, remember to ask for your flu documentation as proof of your vaccination for your Human Resources record.

4) Make an appointment through VNSNY Partners in Care Health Services to go to one of the Mobile Health Management Services locations. Click on one of these geographical areas for a Mobile Health location near you:  Bronx, Brooklyn, Manhattan, Queens, Staten Island and Long Island. An appointment through VNSNY Partners in Care Health Services is required for getting a vaccination and can be requested by calling 212-609-7749 or 212-609-7752 or 212-609-4428. When at the mobile unit, remember to ask for your flu documentation as proof of your vaccination for your Human Resources record.

DECLINATION FORM

If you choose to not receive a flu vaccination, click here to download and fill out a VNSNY Influenza Declination Form.  Submit the filled out declination to one of the methods below.

SUBMITTING YOUR FLU DOCUMENTATION

Please be sure to get a copy of your vaccination documentation as proof for your Human Resources record if you get your flu shot at a location other than the VNSNY Partners in Care offices. 

For Partners in Care employees, please submit your vaccination documentation OR VNSNY Influenza Declination form by doing one of the following:
