Tiffanie Taiste, Clinical Care Manager, VNSNY Care360° Solutions

“My goal is to ensure that each and every patient I make contact with is linked to ongoing supports and services.”

 For Tiffanie, a clinical care manager with the VNSNY’s care management organization, Care360° Solutions, time is of the essence. “Due to our time-limited ability to service clients, each and every case we’re assigned is high priority,” she says.

A social worker with a master’s degree who just celebrated her one-year VNSNY anniversary in September, Tiffanie’s caseload consists primarily of individuals with a dual diagnosis or co-morbidities such as medical, mental health and/or substance use histories. “My clients’ challenges can vary,” she notes, “and may include housing or financial difficulties, home care needs, nutrition issues, mental health needs, or simply the need for social, emotional or family support. My goal is to ensure that each and every patient I make contact with is linked to ongoing supports and services. Connecting them to the right services at the very beginning is key!”

One of Tiffanie’s notable contributions to her team was the creation of a comprehensive city-wide guide to transportation and resources, that’s used to help expedite the referral process for her department’s clients. The guide, which took several months to compile, is now on VNSNY’s shared drive for everyone’s use. It also continues to grow. “Every day, I make certain to ask questions and make connections with new agencies throughout our community that can help contribute to our resource list,” says Tiffanie. “Having resources at our fingertips helps us make the best of the time we spend with clients and their loved ones.”

Earning an ESPRIT award has been a real highlight of her year, she adds. “Sometimes there’s the question of whether or not others truly recognize how hard you work,” she says. “Finding out that your colleagues and administrators have nominated you for such an exemplary award is truly amazing. Team work has always been, and always will be our best work!”
